Regaining Independence Service in West Sussex

Accessing our Regaining Independence Service

If you are living at home but not coping as well as you would like you can contact West Sussex County Council on 01243 642 555. The team will advise you on what steps could be taken to help you and may well arrange for an occupational therapist from West Sussex County Council to visit you at home and complete an assessment with you.

If you are in hospital and need Reablement support when you are discharged, a social care worker will talk to you before you leave about what will be needed when you go home. They will also talk to the occupational therapists from the hospital who will arrange any equipment and adaptations that you will need at home. The social care worker will then make arrangements with our regaining independence team and our regaining independence assistants with be there to support you. Together we will agree the goals you want to work towards and identify how we will support you to achieve them.

Our Regaining Independence Service is free for up to six weeks as long as you are participating in the programme and making progress. At the end of your programme we hope you will be able live life independently without any support.

If you do need support for longer, one of West Sussex County Council’s social care workers will review your needs with you. If you are eligible for Social Care support they will arrange an assessment with one of the trained benefits advisors to identify whether you should pay towards the cost of any on-going service – this will depend on your personal circumstances. What is Reablement?

Reablement is a programme of short-term care designed to help you regain and maintain your independence – giving you the confidence to manage independently. Unlike traditional home care, where carers visit and complete tasks for you, our multi-disciplined team works with you to learn or re-learn the skills you need for everyday life.

Our Reablement service is available to those that have had a spell in hospital or individuals that may be experiencing difficulties living independently at home.  We can help with things such as:

  • Developing confidence to enable safe transfers including getting up from a chair, in and out of bed and on and off the toilet.
  • Providing you and your carers with safety awareness tips to prevent falls or avoidable accidents reducing risks around the home.
  • Support in finding new ways in dealing with everyday tasks including: food and drink preparation, shopping, organising and planning daily routines, using transport, laundry, household administration and community access.
  • Personal care tasks including opportunities to undertake washing, dressing and toileting independently.
  • Working with you to enable you to live life independently. 
  • Signposting to other local services which can support you including Carer Support Services and Age UK.
