From hospital to home: Geoff's journey back to independence
At 89 years old, Geoffrey Bearman values his independence. Living alone since the loss of his beloved wife, Val, in 2019, Geoff has always enjoyed maintaining an active and sociable lifestyle, supported by his close-knit family of children and grandchildren.
When a serious fall at his front doorstep led to a hospital admission, Geoff's future independence was suddenly uncertain. However, thanks to support from ECL at every stage of his recovery journey, he was able to regain his confidence, return home, and continue enjoying the things that matter most to him.
The challenge
After hitting his head during the fall, Geoff was taken to hospital by ambulance. Following medical treatment, he was referred to ECL’s In Reach service, a pilot programme designed to support people leaving hospital by providing a short-term package of care within a care home setting for up to four weeks, with the aim of helping them return home safely and independently.
During his stay, Geoff received a personalised package of care, led by Sharon Shols. Working closely with NHS therapists, social workers and a wider Multi-Disciplinary Team (MDT), Sharon ensured Geoff received the right support at every stage of his recovery. Through her guidance and encouragement, he gradually rebuilt his confidence and regained the skills needed to carry out everyday tasks more independently.
When Geoff was ready to return home, Sharon referred him to ECL's Reablement team to continue his journey towards independence.
Looking back on this period, Geoff remains deeply grateful for the support he received.
Sharon did a brilliant job. I am indebted to ECL for getting me back into my own home.

A pathway back to independence
Following his rehabilitation at the care home, Geoff was able to return to his own home with the support of ECL's Reablement service.
Community Care Assistants visited twice a day, helping him with essential daily routines while encouraging him to regain the skills and confidence needed to live independently.
Rather than creating long-term dependency, the reablement support focused on helping Geoff do as much as possible for himself. With encouragement and practical assistance, he steadily regained the ability to carry out everyday tasks independently, including preparing meals and maintaining his daily routines.
By the end of the reablement period, Geoff had made significant progress and was able to continue living safely and independently at home. While he had successfully regained much of his independence, he chose to arrange and privately fund ongoing domiciliary care visits at the start and end of each day. This additional support provides reassurance for both Geoff and his family, giving him confidence to continue living independently while knowing help is close at hand when needed.
Continuing the journey with ECL
As Geoff settled back into life at home, his daughter felt he would benefit from opportunities to socialise and connect with others.
Having already experienced the positive impact of ECL's In Reach and Reablement services, Geoff and his family were delighted to discover ECL Chelmsford - Charles Dickens Day Centre.
Following an introductory visit, Geoff immediately connected with both staff and customers, particularly Support Worker, Nikki Young. It quickly became clear that the centre was the perfect fit.
As Geoff does not qualify for support funded by the local authority, he chose to attend the centre as a self-funding customer, investing in a service that helps him stay active, socially connected and engaged in his local community.
Today, Geoff attends every week and has become a much-loved member of the centre. Known for his quick wit, mischievous sense of humour and love of conversation, he enjoys the friendships and sense of belonging that have become an important part of his routine.
"It's the highlight of my week."
More than care
For Geoff, the support provided by ECL has gone far beyond practical assistance.
Support Worker, Nikki has helped him create a personal memory book documenting his life story, giving him a meaningful way to reflect on and share treasured memories with others.
Geoff's lively personality has made him a popular customer at the centre, where his love of banter is warmly welcomed by both staff and fellow customers. So much so, that he even penned a playful poem for the team (pictured), a heartfelt and humorous tribute that reflects the strong friendships and genuine connections he has developed at the centre.
These projects have become a source of pride for Geoff and a wonderful opportunity to celebrate and share his past and present experiences with the people around him.

A continuing journey
From hospital support and rehabilitation through to reablement and ongoing community engagement, ECL has been there at each stage of Geoff's journey.
What began as recovery from a serious fall has become a story about regaining independence, rebuilding confidence and staying connected.
Today, Geoff continues to live independently at home and enjoys an active social life through his weekly visits to ECL's Day Centre. After first experiencing ECL's support during his recovery journey, Geoff and his family have chosen to continue that relationship through ECL's self-funded day services, enabling him to stay connected, engaged and doing the things he enjoys most.
