Find out more about ECL Reablement care, and how our team will support you and/or your loved one.

Welcome

Following an assessment, you have been recommended as needing a period of intensive short-term support for tasks around the home. You may have heard this being called ‘Reablement’.

What happens now?

You may have been referred to us by your GP, hospital, social worker or family after  a spell of ill health as you require care and support to get you back up on your feet.

A social care or health professional, with support from an ECL Trusted Assessor, has decided that this type of care is suitable for you. Once a referral has been made your service will usually start within 24 hours.

If you are in hospital, sometimes our Trusted Assessor will visit you there to help you make arrangements for your return home.

One of our Trusted Assessors will visit you at home to undertake a customer support assessment with you. This means we will discuss what you can do, what you would like to achieve using the service and how we can support you to ensure you get the most from our help.

Once we have found out what support you need, we can jointly agree some goals for you to work towards. This is nothing to worry about –it’s just a way of tracking how much progress you make with our support.

We use your goals to make sure you have the right support in place to make as much progress as you possibly can.

To ensure our staff have real time and up to date information, we use electronic care records, this also helps protect the environment with reduced printing.

Who might visit your home?

Trusted Assessor

A Trusted Assessor will be the first ECL team member you meet with. They will assess your needs and work with you to create a plan of care and support that you agree to. They will visit you regularly throughout your programme of care to ensure you’re on the right road to recovery.

Community Care Assistant

This team member will help you with general personal care. This might include washing, dressing, personal care or medication. They’ll help you with your agreed needs as outlined in your support plan.

Making progress

During the time that ECL supports you we will review the progress you are making.

It’s likely that you will see the same key worker (Trusted Assessor) throughout our time with you. They will ensure that the support we give is tailored to meeting your set goals.

The idea is for you to feel happy with them in your home and comfortable enough to ask for more help if you need it.

Your Trusted Assessor may suggest physiotherapy, occupational therapy or meeting new people and trying new activities at one of our day services, if they feel you would benefit from these services.

What happens after my short-term support from ECL ends?

Please ask an ECL team member for information about ongoing care, they will be more than happy to provide some options for you. It is good to note that your local authority can arrange a financial assessment for you if you believe you qualify for financial support for the funding of your ongoing care
